This form may be used for authorization of copy express and/or print shop services. Please fill in the number of originals and the number of copies per each original. Also, check other appropriate items desired and describe additional services or special instructions. On reprint orders a sample should be attached (i.e., letterhead, envelopes, forms).

Copy Center provides duplicating services of line originals to be reproduced in black ink on white or color bond paper material. And may involve collating and stapling or binding of manuals or reports.

Routine jobs — those with 50 originals or fewer reproduced in quantities of 5 to 300 copies per original — may be hand carried or mailed to Copy Express Plus, located on the fourth floor of the Medical School Building in room 406L.

Jobs with 51 + originals or that require book binding are scheduled by order of their arrival. Although turnaround time for these jobs usually is seven to 10 workdays, it is recommended that completion dates for large projects be discussed with the copy center production supervisor.

Print Shop provides services requiring processes that are not generally achieved through conventional copy center (xerographic) production. For example, print shop work may include color inks, halftone photo printing, reproduction of complex graphic designs in sizes of 3x5 up to 19x25 inches, special paper materials or magazine type binding.

Turnaround time is 5 to 10 workdays on routine orders — bulletins, forms, newsletters, business cards, jobs whose finished size is 3x5 to 8.5x14 inches. Complex graphic designs requiring multicolor printing, photo manuals, magazines, directories should be scheduled as far in advance of deadlines as possible with the print shop production supervisor.

The Print Shop offers desktop publishing in both PC and Macintosh platforms. Products include high — quality color publications, booklets, brochures, flyers, logo designs and a side variety of printed materials. Typographic support is offered in developing manuals, publications, and forms. Please consult with a Printing Services Supervisor for scheduling and estimates.

Normal turnaround time is 10 working days. Printing Services has the ability to rush your order with the design and/or printing process. For rush orders please consult with a Printing Services Supervisor for scheduling and any additional cost.
